Information we collect

Eudaven may collect account information, intake responses, treatment history, fulfillment status, and support interactions needed to operate the platform and coordinate clinician-led care.

How information is used

We use submitted information to support eligibility review, patient communication, prescription workflows, payment operations, and service improvement. Access is limited to systems and teammates involved in delivering the service.

Current status
